Fatty Liver Disease Management in Dhaka

Fatty liver disease (NAFLD and NASH) is now one of the most common liver conditions in Bangladesh. Most cases are not surgical — they need lifestyle change, treatment of associated conditions, and structured monitoring. Dr. Arefin emphasises a non-surgical first approach and reserves surgery for the small subset of patients who develop complications.

    What is fatty liver disease

Fat accumulation in liver cells, usually associated with obesity, type-2 diabetes, high cholesterol, or alcohol. Most patients have no symptoms; the diagnosis is often incidental on ultrasound. A subset progresses to NASH (inflammation), fibrosis, and eventually cirrhosis.

Non-surgical management — the first line

    • Weight loss of 5–10% of body weight reverses most early disease
    • Tight control of diabetes and lipids
    • Regular physical activity — 150 minutes per week
    • Avoidance of alcohol and unnecessary hepatotoxic medication
    • Ultrasound and FibroScan monitoring at intervals decided by liver-fibrosis stage

    When surgery becomes necessary

Surgery is needed only when fatty liver progresses to advanced complications — large hepatocellular carcinoma needing resection, decompensated cirrhosis needing transplant, or coexisting symptomatic gallstones. Dr. Arefin coordinates surgical care with hepatologist follow-up.
