    Cholecystitis Surgery Dhaka

Cholecystitis is inflammation of the gallbladder, almost always caused by gallstones obstructing the cystic duct. Both acute and chronic cholecystitis are best treated by laparoscopic cholecystectomy. Dr. Murshidul Arefin manages both elective and emergency cholecystitis cases at Popular Medical College Hospital, Dhanmondi.

    Acute versus chronic cholecystitis

Acute cholecystitis presents with sudden severe right-upper-abdominal pain, fever, and a tender, palpable gallbladder (positive Murphy's sign). Chronic cholecystitis presents with recurrent biliary colic and fatty-food intolerance over months. Both are caused by gallstone-induced cystic-duct obstruction in >95% of cases.

    Diagnosis and timing of surgery

Ultrasound confirms a thickened (>3 mm) gallbladder wall, pericholecystic fluid, and stones. Modern guidelines (Tokyo Guidelines) recommend early laparoscopic cholecystectomy within 72 hours of acute presentation — this is safer than the old practice of cooling down with antibiotics and operating 6 weeks later. Dr. Arefin follows this evidence-based pathway whenever the patient is stable.

    Surgical approach

The standard operation is laparoscopic cholecystectomy, often with placement of a sub-hepatic drain for 24–48 hours when significant inflammation is present. In severe inflammation that obscures Calot's triangle, Dr. Arefin uses a sub-total cholecystectomy — removing the gallbladder dome and stones while leaving a small cuff. This is far safer than risking bile-duct injury and is fully accepted modern practice.

    Recovery

Hospital stay is typically 1–3 days for acute cases and same-day for chronic cases. Patients return to office work in 1 week and to manual work in 2–3 weeks. Untreated cholecystitis can progress to empyema, gangrene, or perforation — early surgery is far safer than delay. Book a consultation as soon as symptoms appear.
